Conveniently located on the state border in Longmeadow, MA, Safe Harbors offers services to seniors in both Western Massachusetts and Connecticut.

Our services include, but are not limited to the following. If there is something else that you have in mind, just ask. We’re happy to help in any way.

• Represent or accompany family members at discharge planning meetings.

• Assist with selection of short-term rehab options based on patient and family criteria.

• Identify alternatives for continuing care including home care, day care, and residential care settings.

• Communicate with Medicare and private insurance companies to confirm coverage for various types of care.

• Contact home care agencies to initiate clinical assessment and scheduling of nursing and/or nursing aide visits.

• Moderate continuing care discussions among family members.

• Visit individually with seniors to obtain their perspective on the type and quantity of care they would prefer.

• Recommend private and agency home care providers.

• Identify independent living, assisted living, or long-term care facilities that suit your personality, meet your care objectives, and work within your budget.

• Schedule appointments for seniors and/ or family members to visit and evaluate facilities.

• Assist with gathering required medical documentation and completing application forms for residential settings.

• Accompany seniors and/or their family members to facility visits.

• Provide guidance regarding the special needs of memory impaired seniors and the services available to them.

• Update out-of-town siblings on changes in care needs and continuing care options.

• Guide out-of-town adult children in selecting safe care environments for aging parents who wish to remain in their home-town area.

• Make referrals for contingent services to local professionals such as elder care attorneys, physical/ occupational therapists, dementia specialists, transportation services, etc.

Our fees are billed hourly, allowing you the flexibility to utilize our services only as needed.
